In order to receive AHA ECC cards at no cost to BREMSS EMS Providers, this form must be completed and approved 15 days prior to the intended training date. No cost cards will only be provided to personnel actively associated with a BREMSS EMS agency.

The agency must perform EMS related operations in order to qualify. 

Examples of qualifying agencies: EMS transport services (Public or Private), Volunteer Fire Agencies, Career Fire Agencies, EMS Dispatch Centers, Air Medical Services
